On behalf of Training Command, J7 Training Staff, J6 Policy Staff, the Medical Training Team and Medical Force Manual Development Group, the following changes have been made to medical policy and progression.
Tier Updates:
-
Tier I: Basic first aid, airway management, triage, splints, painkillers, CPR, ammonium carbonate.

Tier II: Personal Aid Kits, IV bags, blood transfusion kits, King LT airway tools, pulse oximeters, nitroglycerin.

Tier III: Advanced tools/medications (e.g., TXA, EACA, norepinephrine, epinephrine, AED(-X)).
Notable Changes:
-
TXA, EACA, norepinephrine, phenylephrine moved from Tier IV to Tier II/III.

Pulse oximeter moved from Tier III to Tier II.

Epinephrine (wake-up chance) moved from Tier IV to Tier I.
-
Cardiac rhythm meds (epinephrine, amiodarone, lidocaine) now Tier III.
New skill added:
-
Administer nitroglycerin to reduce blood transfusion time.
-
Limited to 6 doses every 6 minutes to avoid overdose.
-
Side effects: increased bleeding, heart rate, reduced blood pressure.
Loadout Policy:
-
Squad medics: Minimum 2 doses, recommended 8 doses.
Advanced Heart Rhythms:
-
Introduced 3 new rhythms (total 5), adding complexity for medics with AED(-X).

Unless otherwise stated these changes are effective immediately and the medical grace period for new skills is 30 days.
